Registration includes admission to the workshop, course workbook and indicated meals.
All registrants must pay the full registration fee; one-day registration is not available. Spouse/accompanying guests (healthcare professionals) who wish to attend the workshop must register.

Important Cancellation Deadlines

  • Full refunds for registration - cancellation notice must be received by July 4, 2014.
  • 50% refund  - cancellation notice must be received by July 19, 2014.
  • No refunds will be given if cancellation notice is received after July 19, 2014, or for those who fail to attend.

All refunds are subject to a $25 processing fee.
All cancellations must be received in writing to be considered for refund.
