As a new patient you will be asked to complete patient registration forms. These will be sent to you if time permits. We ask that you bring your insurance information with you to your appointment.

The office visit is designed to be an educational experience that allows you to make an informed decision regarding your care. Initially you will undergo a complete examination. This will be followed by a thorough explanation of your condition and the recommended treatment.

Facilities

Dr. Yessenow currently has operating privileges at most hospitals in Northwest Indiana. In addition he is on staff at many outpatient surgery centers. This allows for maximum patient convenience in an enviroment that suits their individual needs. All attempts are made to provide the patient with a comfortable, safe, and efficient operating experience.
