Quick fixes and fads come and go. Sound surgical principles deliver the most viable and lasting results.There are many new gadgets, "non-invasive" procedures, 15 minute facelifts, miracle creams, and painless skin tighteners. I believe the makers of such products, and some of the "physicians" who "endorse," or "highly recommend," such products or procedures prey shamelessly on the average consumer. THERE IS NO LEGITIMATE SCIENTIFIC STUDY THAT DEMONSTRATES ANY REVERSAL IN AGE RELATED WRINKLES, CREASES, OR DIVOTS WITH THE USE OF ANY CREAM, MAGIC SPELL, OR POTION, NONE, PERIOD. This is not to say that most consumers are stupid, or uninformed. Most are fed poor quality pre-intervention and post-intervention photographs with changes in lighting, background, focus (or complete absence of focus). People are trained to pick up on this, they see a change for the better, and attribute it to a miracle treatment. There has also been a media push to embellish medical advances. This misleads the average patient into believing that "new" techniques are somehow better, without being as invasive, with the added benefit of less down-time. They are fed non-sense about "minimally invasive plastic surgery." They believe it. To understand why the term is laughable, we need to go back to the origins of minimally invasive surgery. The entire purpose of laparoscopic surgery was, and is two-fold. Using very small incisions to perform intra-abdominal, intra-pelvic, or intra-thoracic surgery allowed patients to return to activity quicker because most pain caused by such surgery is related to the incisions across skin and muscle. The second reason, is obviously to improve the cosmetic appearance of the scars by making scars tiny. Plastic surgery, however, for the most part focuses on skin. There may be too much, too little, it may need to be reshaped, etc. Plastic surgeons operate on skin. To say that an operation that is designed to remove, add, or reshape skin can be performed by not making incisions in the skin is downright silly. For any plastic surgeon to endorse a technique that promises long lasting results in a fraction of the time, with no downtime implies one of two things. The surgeon is a fool, or the surgeon is more interested in making money than in his or her work, and in the satisfaction and ultimate appearance of the patient. There are cases, breast augmentation for instance, or a closed rhinoplasty, or an endoscopy browlift, where making the incision smaller may yield a more pleasing result. For the most part, however, operations that have been around for 30 years, are around for a reason. The same cannot be said of the gadgets sold to "doctors" two years ago for hundreds of thousands of dollars, that are now available for a few thousand bucks as refurbished resale items. Results don’t lie, and once word gets around people know what not to pay for. The trouble is that "new" and "improved" treatments are being churned out so quick, those who have them done first don’t have the luxury of the truth.Having said this, my practice philosophy is rooted in honesty, humility, and quality. I would never promise the stars and the moon; no matter how god a job I thought I may be able to do. I have always given a fair assessment of the situation, presented working alternatives, discussed most of the unfavorable events that could have possibly occurred and made sure that my treatment plans were in line with my patient’s goals, and expectations. I have never thought myself too good or proud to refer to someone who may have more expertise in a given area, rather than operate on a patient for a quick buck without regard for potential problems. Finally, I have stood by my work, and I am happy to display any case when allowed to do so by my patients. I will strive to preserve the honesty, humility, and quality of my practice as long as I am caring for patients.

I'd need a bit more information to get an idea of how to proceed. What is your height and weight? What is your HgbA1C? Both procedures carry risk, especially in the face of poorly controlled diabetes but buttock augmentation involves a foreign body (implant itself), which makes it more prone to complications.

It sounds like your daughter suffers from significant symptoms. The most important thing to consider is possible additional growth even after reduction or the enlargement related to pregnancy in the future. Both may necessitate additional reduction. That, however, is a moot point. The problem causes her not only physical discomfort but what you're describing as psycho-social distress. Take her to see a reputable, board certified plastic surgeon in your area.

1. Unless there is a specific problem with the unaffected breast there is no reason to touch it. 2. Both sientra and allergan have 2 year warranties that cover capsular contracture surgery in addition to providing implant replacements. 3. There is no definitive answer as to what causes capsular contracture. It is a fibrous reaction. It can be a result of left over blood, clinical or subclinical infection, the use of smooth vs. Textured implants, the selection of the pocket, the use of antibiotic solution or betadine to wash out the pocket. Yes the problem may recur even if all the factors I just mentioned are optimized.

I think you are asking the wrong question. A surgeon can do 1000 facelifts, if they all look like a Picasso, how is that to your advantage? As I recall Michelangelo was not a painter first, but he did a pretty bang-up job, with all its imperfections, on the Sistine Chapel. It would be to your advantage to ask whomever you consult with to show you their work.

For fat transfer, there is no drainage tube (otherwise you'd not only lose fat but introduce a portal for infection). I personally do not use drains for breast augmentation, but there are surgeons who prefer leaving a drain behind.

It's not a matter of how much can be safely transferred, you can transfer more than 420 cc to match current size, it's a matter of how much is going to stay. Instant gratification is the hallmark of today's society. Unfortunately when it comes to breasts, you will likely have to stage the transfer, meaning you'll have to do a transfer from the abdomen, then maybe a transfer from the waist. This is because only a portion of what is put in (I mean fat) actually stays. You'll have to wait 4 months to determine how much fat has been retained. Generally 40-50% stays when a large amount is put in.

Regardless of how much of a "right" and "healthy" patient you may be there is no question that prolonged operative duration increases the risk of surgical site infections as well as deep venous thromobosis (lower leg clots that may go to the lung circulation). The question should be, is it safer to split the procedures up or to do them together. Once again, there is no question that it is safer to split them up, especially since liposuction can be done under local or sedation. How long will it take you surgeon to do the procedure?

There are surgeons/companies who attempt to trademark procedures and engage in branding. The method you ask about has been around in some form or another for around a hundred years. I personally did most of my breast reduction training using such a technique, and though I am very adept at horizontal and periareolar incision only breast reduction, it has some definite disadvantages. I've come full circle to include the vertical incision; the gain in shape far outweighs any claimed benefit of a less conspicuous incision.
