Ask a Surgeon
Dr. Kenneth Francis participates in the ASPS Ask A Surgeon service. View responses to public questions below.
Sudden pain for contracture?
Breast Implant RevisionMember Response:
Dear LisaGee, Capsular contractures do not happen over a couple of days. There is likely a recurrent seroma. Although the natural history of ALCL is usually on the order of 8 years or more, there are reports of shorter time frames for developing the disease. To date, it has almost exclusively been found in textured implants (macrotextured mostly, which Mentor is not). You need to return to your surgeon for an examination and guidance. Good luck!
Challenging case
Buttock ImplantsMember Response:
Dear Nessa, There are many plastic surgeons that would remove your buttock implants. Keep searching. Removal is not the concern. Insertion is.
Breast Lift with Implants
Breast AugmentationMember Response:
Busysosis You need to speak to your surgeon, the one you put trust in to do your surgery.
Is it worth it
Breast ReductionMember Response:
Dear anon, It's worth it.
Symmastia Repair
Breast Implant RevisionMember Response:
Stacy, I don't often answer the questions here, but when I see a need to comment, I do. I don't know what your surgeon is saying, but, you do not have synmastia and your breasts look great in my humble opinion. Are they close to the midline? Yes. But, there is a clear delineation between each breast, that is even more apparent now then it was pre surgery. Not sure what you're looking for. I would call it great cleavage!
Hyaluronidase
Injectable FillersMember Response:
Angel, Sometimes it takes several sessions of hyaluronidase injections to completely dissolve the filler. You may just need more. Try another session of injection to see if you still respond to it. If not, then sometimes the lumpy can be corrected with reinjection of HA's. Good luck!
Hole in belly button
Tummy TuckMember Response:
Dear cc, You should be examined by a physician. If you have a primary care MD or GYN, you might want to start there. They can refer you on to a plastic surgeon if necessary. It could be an exposed, or rejecting stitch from your TT, although you'd probably be having some drainage from your BB if that was the case. Good luck!
Lower eyelid surgery and cheek fat transfer
Eyelid SurgeryMember Response:
Dear aj, Without an exam it's a little difficult to be sure, but it is likely a ribbon of fat that is creating the roll, or "some sort of vein." This sometimes happens with fat transfer to the lower lids, and is what makes the procedure delicate. Massaging sometimes helps. If it doesn't, your surgeon might consider a very light cortisone injection. I have had some success with microliposuction for the scenario. If none of these are effective, sometimes it needs to be surgically removed. Good luck!
16 Months Post-op Tummy Tuck Swelling
Tummy TuckMember Response:
Dear Marshid, Unless something is very unclear (like the laxity of your abdominal skin) in these photos, it appears that you were never a candidate for a tummy tuck in the first place. Your abdominal contour was phenomenal to begin with and a little bit of liposuction would probably have been the proper recommendation. So either you were sold a bill of goods, or you demanded a procedure that you did not need. Something is missing in the story. This is further evidenced by the vertical scar below your navel. Unheard of, and NEVER EVER performed in my practice of 30 yrs. I would disagree with my colleague that you now have fat creating the "pooch." You had very little fat to begin with. It is continued edema (swelling) compounded by your mesotherapy injections, which cause inflammation, and guess what?, more swelling. So, liposuction is not the answer. Leave things alone. The edema will improve with time and exercise. Good luck!
Breast Asymmetry
Breast AugmentationMember Response:
Dear Ana, According to the ASPS statistics over 100,000 breast reductions were performed in the US alone in 2020. The procedure is common. It is safe. There are risks of course, as there are with any surgery. The options for correcting your breast asymmetry may be several, but please do not rule out reduction of the larger breast based on internet research. Good luck!
Breast lift
Breast Implant RemovalMember Response:
Breast implant removal and breast lift performed simultaneously is generally not a very complicated procedure. However, it can become complicated if your breast implants are silicone gel and are leaking, or if you have very severe or calcified breast implant capsules. Seek consultation with an American Board of Plastic Surgery certified plastic surgeon for an examination, ultrasound/MRI if indicated and discussion of your options. Good luck!
Lip Scar tissue removal/ lip realignment
Lip Augmentation/EnhancementMember Response:
Dear palakk15, Once a scar is present it is present in some form for life. There may be options for reducing it's visibility. Seek consultation with an American Board of Plastic Surgery certified plastic surgeon for an examination and discussion of your options. Good luck!
How to find information on my implants?
Breast AugmentationMember Response:
Dear Shiena, With all due respect to the other responder, it is not as simple as that. In NY medical records are required to be kept for 7 years only. Each state differs slightly. Your best bet would be to contact the customer service departments of the 4 main manufacturers in this country, Allergan, Mentor, Ideal and Sientra to see if they have any record of your implants. Good luck!
Pulmonary embolism
Tummy TuckMember Response:
Dear sabby, Yes, with a history of PE, at age 43, undergoing a prolonged surgery, you would be considered in the highest risk category for another blood clotting event. This would require anticoagulation during and after your surgery. Many surgeons would decline you as a patient. Since your coagulation workup took place over 10 years ago it would be wise to see a hematologist and have it repeated since advances in coagulopathy diagnosis have been made since then. Also, the hematologist would become involved with your anticoagulation protocol for surgery. Good luck!
irregular heart beats
Chin SurgeryMember Response:
Dear Elizabeth, You need to immediately call your plastic surgeon and inform him/her of your symptoms and concerns. They may tell you to get to your primary care doctor or cardiologist, but they need to know what is happening with you. Good luck!
Breastfeeding
Breast ReductionMember Response:
Dear Amanda, The likelihood of you being able to breast feed is pretty high after inverted T breast reduction. Good luck!
Areola stretched after 5weeks PO
Breast AugmentationMember Response:
Dear Angelika, Your areolae do appear asymmetric. However, it is too early in your recovery/healing process to consider revision. Continue follow up with your plastic surgeon to determine the correct time for a revision. Plan on about 6 months. Good luck!
Buttocks Asymmetry
Body ContouringMember Response:
Dear Ritah, Buttock asymmetries can be corrected, or at least improved in a variety of ways, depending on what is causing the asymmetry. Seek consultation with an American Board of Plastic Surgery certified plastic surgeon for an examination and discussion of your options. Good luck!
Question about modified buttock lift after weight loss proceedure-cannot find before/after pics
Buttock Lift with AugmentationMember Response:
Dear Jill, An upper buttock lift is not likely to transmit all the way down to your lower buttocks, even with the flap augmentation. An option might be direct removal of skin along your lower buttock creases. Fat transfer might then enhance the results. Good luck!
Is this normal post-op
RhinoplastyMember Response:
Dear BJ, Healing after rhinoplasty surgery takes at least a year. Continue follow up with your surgeon and follow his/her guidance. Good luck!
