Thomas G. Fiala, MD, MBA, CPE, is an ASPS Member plastic surgeon who is board certified by The American Board of Plastic Surgery® and trained specifically in plastic surgery. ASPS members operate only in accredited medical facilities, adhere to a strict code of ethics and fulfill continuing medical education requirements in plastic surgery, including training in patient safety techniques. Meet Dr. Thomas Fiala

Dr. Fiala and the entire staff are committed to fostering an atmosphere where patients feel comfortable to ask questions or express concerns. Each patient is treated with respect, dignity and understanding.

The combination of outstanding patient education, proven, effective surgical techniques and the highest medical and safety standards deliver a predictable and natural looking outcome. Perhaps most importantly, we will never promise a result that cannot be delivered.

Dr. Fiala is the Past President of the Florida Society of Plastic Surgeons, and a voluntary member of faculty of the College of Medicine, University of Central Florida.   He is active on multiple committees with ASPS and ASAPS, and is Vice-Chair of the Cosmetic Section, In-Service Exam Committee.

Your Consultation

Dr. Fiala likes to meet and have a detailed discussion with all of his patients - so that he can fully understand the patient's goals. This in-depth session allows an opportunity for a detailed explanation of the various surgical options, and their benefits and trade-offs. We feel that a well-informed patient is more likely to have a smooth recovery and good outcome. Pre-operative education is a big part of preparing for surgery. Consultations are typically scheduled on Tuesdays and Thursdays of each week. Special arrangements can be made for out-of-town patients.

Boob job on one side?

Hi. Yes, it certainly is possible to have a breast reduction on just the larger side! That might work quite well for you, as long as the smaller side is not droopy. This has the advantage of just working with your natural tissue, and not requiring any implants.

Is it safe

That's a lot of surgery at one time. Personally, it's much more than I would feel comfortable doing together. Here in Florida, if we are doing abdominoplasty (tummy tuck), we are limited on the amount of liposuction we can do simultaneously - but different states have different rules.

Hi. That's a big question - and usually that's exactly what is discussed at a consultation. It sounds like you are ready to take that step. If you are having back issues related to the breast weight, you might do better with a breast reduction rather than a breast lift. Fortunately, most patients do really well with breast reductions, and are back at work within 2 weeks, and feeling more comfortable, and more proportional. They are some of the happiest patients in plastic surgery! While all surgery has risks, most patients do well, as long as they are in good general health, aare not smokers, and follow the surgeon's post-operative directions.

Hello Curious Cat: Yes, when we do a breast reduction, the breast is automatically "lifted" at the same time. You don't have to ask for a lift as a second stage. The extra skin and breast tissue are removed together. All the best!

Breast augmentation with fat

You would need an in-person consultation to determine exactly what is needed. But hypothetically, you could liposuction fat from the thighs and flanks (love handles) and transfer some of it to the breasts. It may take more than 1 surgery to get the size and shape you want, and typically you wouldn't get the rounded breast shape that one can achieve with implants. So, there are pluses and minuses to the technique - your surgeon can discuss with you in detail, versus using an implant.

Yes - this is possible. Depending on the situation, you could potentially reduce, lift or augment one side to get a better match with the other. For example, if you had a C cup breast that you liked on one side, and a droopy DD on the other side - the DD could be reduced & lifted to be a closer match.

Hi Suzann. More important than your calendar age is your state of overall health. Do you have any problems with your heart or lungs? Are you a smoker? A diabetic? And so on. If you are in good shape, it should be possible for you to have a tummy tuck with a baseline level of risk. Best to discuss your particulars with a local Board-certified Plastic Surgeon.

Hi. Yes, fear of anesthesia is a big issue for many people. In our center, we only use board-certified MD anesthesiologists for all major surgeries. They are the most experienced & highly trained. Make sure you ask who your anesthesia provider will be, as this is not the case everywhere...

Hi. You would be best to call your insurance carrier, and pose the question about coverage to them. Ask them to explain their coverage guidelines for "unilateral breast reduction". I would hope that, just like a typical breast reduction patient, that you would be able to have this covered, as a reconstructive procedure. They may, however, rule that it is a cosmetic problem, and say you have to self-pay.

This is a well-known situation for most plastic surgeons, known as a dynamic flexion deformity. Putting the implant over the muscle fixes this problem, but increases the issues & risks for other problems: hardness (capsular contracture) and implant visibility and ripples, particular for women who diet down to a very low body fat percentage for competitions. I always ask active body builders what implant position they would prefer, after explaining these choices.

Unfortunately, the vast majority of insurance plans have no desire to cover cosmetic surgery, like a tummy tuck. It is typically a self-pay situation. Many practices do offer financing options, though.

Costs for surgery can vary pretty widely around the country, even for the same procedure. Here in Orlando, silicone gel breast augmentation is around $6000-$6500, less for standard saline implants. While there are always "cheap" options, don't gamble with your safety. Have a board-certified plastic surgeon, a licensed anesthesia provider, and an accredited facility for your procedure.

Breast reduction patients are generally some of the happiest patients in plastic surgery. Even though the reduction means some incisions on the breasts, most women feel much better in terms of their posture, shoulder & neck discomfort, difficulty with exercise & getting clothes to fit, and general self-image. It is not uncommon to do this operation for young women, before they've had children. Try to get your weight under 200 lbs prior to surgery. All the best,
