Lymphedema TreatmentMember Response:
Hi Linda, I don’t see any specifics related to your Lymphedema on this post. Our group performs several procedures for arm Lymphedema as a result of mastectomy and node harvest for breast cancer. We do Vascularized Lymph Node Transfer where we transplant nodes from your groin to replace the ones removed under the arm, as well as Lymphovenous Bypass where we identify the lymphatic channels in the arm that are blocked since the nodes they originally drained to were removed and instead connect/bypass them to an adjacent vein instead (like clearing a wreck causing a traffic jam). There are many details to go over regarding your individual case (too much for a 1000 character post limit), but I’m happy to discuss with you over a quick phone call to see if you’re a candidate. With proper selection of patients most likely to benefit from surgery, our group has had fantastic results solving this difficult, chronic and debilitating problem. Best, Dr Reis

Hi, so sorry to hear that your less than satisfied with your breast reconstruction outcome. This is a great question and has two “right” answers depending on your goals. Flap reconstruction is definitely the way to go either way, since implant reconstruction and radiation mix about as well as oil and water. The options and main question as you stated is whether to just remove the implant on the radiated side and reconstruct with flap(s) or whether to have a “preventative mastectomy” on the other side as well along with reconstruction on both. There’s no right answer across the board, it really depends on your preferences. The two things that you need to consider when deciding which path you prefer, are: #1 that bilateral reconstruction generally leads to better reconstruction outcomes, and #2 that with preventative mastectomy there’s an added benefit of not having to worry about the “good breast” and whether this may one day develop an issue.
