Meet Dr. James Chappell

Dr. Chappell and his staff provide their patients with personalized, experienced, technically advanced and ethical care. Dr. Chappell has dedicated himself to excellence in plastic surgery since 1998 and realizes that a complete understanding of your unique needs will aid in providing you the most successful surgical outcome. He spends a great deal of time in personal consultation with each patient in order to best determine how he can aid them towards their goal of an improved sense of their body.  Dr. Chappell uses only state-of-the-art technology and techniques to give you the best results.  He and his staff are courteous, welcoming and gracious, and patients are treated to a wonderful experience from the first telephone call (or email).  Dr. Chappell also has a spa-like setting in which his aestheticians, Kris Lawrence and Lauri Leight, provide medical-grade skin care in a soothing setting to help patients forget about the real world for a while. In 2013 Dr. Chappell introduced Coolsculpting to the Annapolis area, adding non-surgical, targeted treatments for problem fatty areas.  Since that time hundreds of patients have successfully undergone no-downtime procedures and seen great results.

Your Consultation

The patient meets with Dr. Chappell one on one from the very first visit. He takes the time to listen to the patient's concerns as well as performing a meticulous physical exam, keeping the patient's dignity in mind at all times. In addressing the patient's concerns, he keeps in mind the patient as a whole. With patient safety being of utmost importance, Dr. Chappell addresses the patient's medical history as well as the patient's emotional well being. You can be assured that Dr. Chappell and his staff will maintain the strictest confidentiality. The day of the initial consultation, Dr. Chappell's patients are able to discuss the risks and benefits of surgery or procedure, review photos, have digital imaging done, and review the costs in order to make an informed decision. You will leave the office feeling confident and educated with detailed written surgical instructions, brochures and cost analysis.

Facilities

River Reach Outpatient Surgery Center: To keep consistent with a private and personalized atmosphere, Dr. Chappell performs most of his surgeries at the same facility where the patient has already had his or her initial consult, and he or she is familiar with the environment and educated staff. In order to provide the patient with the current safety standards, Dr. Chappell's state of the art facility is fully accredited by the American Association for Accreditation of Ambulatory Surgery Facilities, Inc. (AAAASF), is licensed by the State of Maryland, and Medicare certified.

How do fat cells really work?

I would add that liposuction is targeted fat removal from problem areas, but patients need to remember that though they won’t generally gain weight back in those same spots, if one eats 100 calories and doesn’t burn it off it is then going to go to it’s next favorite place (butt, breasts, neck, thighs…). I agree with what Dr. Weinstein wrote.

BIA ALCL after implant removal

Total removal of the capsule should leave little to no chance of developing BIA-ALCL. That said, it is cancer, we still don't know everything about it because it is a relatively new phenomenon, and unfortunately I suppose anything is possible. If BIA-ALCL suspected complete capsule removal (capsulectomy), and removal of any other suspicious looking tissue, is called for.

Using peroxide

Peroxide and Betadine fight infection but they also inhibit healing. Your surgeon knows you best. From a hypothetical standpoint if there is no longer infection I personally recommend hydrogel, Silvasorb or a calcium alginate dressing. But I cannot say what I would do in your case as I do not know you. Best of luck to you.

Consider a company called Carecredit ( I do not have any financial or other commitment to them but do offer it in my practice). If your surgeon takes it you won’t need to make a payment for 6-12 months. You can save your money up for those 6-12 months and then spread out your $5k over the term of the loan.

Explant

Probably seromas (fluid collections) in the same spaces from which the implants were removed. I usually remove most if not all of the scar tissue (what we call a “capsule”) around the implants when I remove them for this very reason. Costs more, requires placement of drains and takes longer, but helps prevent this. I find it unlikely that a properly trained and experienced plastic surgeon who sets out to “remove implants” wouldn’t feel/see another remaining implant in the same place, but I wasn’t there.

Will insurance cover this procedure?

Your specific insurance policy dictates the terms of coverage and a competent surgical scheduler in an experienced board certified plastic surgeon’s office can work with you to help establish if you will get it covered. Of course you can call the insurance company and they can tell you too. Assuming you will have a minimum of about 1.1 or more pounds removed per breast and have relevant symptoms that you have reported to your primary doctor (and possibly have had physical therapy or chiropractic care) you may get the surgery covered.

Abdominoplasty with upper abdominal liposuction

Judicious amounts of uppermost abdominal liposuction done at the same time as abdominoplasty in selective patients is definitely reasonable and safe in the hands of a well trained and experienced board certified plastic surgeon in my opinion.

I agree with Dr. Pober. A board-certified plastic surgeon could perform panniculectomy or tummy tuck. Unless you have chronic wounds or rashes under the overhanging skin I doubt any insurance company would pay for it. I wish you all the best.

Best to consult with a board certified plastic surgeon to see which procedure or procedures are appropriate to address your concerns and desires. In addition, as surgeons we only want to do procedures on patients who are medically appropriate for said procedures. As for correct weight, BMI <30 is better, <25 is best. Good luck to you.

On the face of it, combining spinal fusion surgery with a MM just doesn’t sound right (positioning issues, what happens if you had any complications from any of the surgeries and needed emergency corrective surgery, infection issues possibly wrecking your fusion, ...). Too many variables. I would get the spine done and later, if your neurosurgeon clears you, CAREFULLY get the other stuff done. Patient safety is paramount.
