Agree with the comments above. It is not a question of who can perform the procedure, but whether you are a good candidate. I can tell you I have not encountered a patient yet who is a candidate for a crescent lift after implant removal, but it depends on your age, skin quality, breast shape, and implant size, among other factors. If you have sought several opinions and they have advised against it, perhaps you should heed the advice rather than searching for someone to do what you ask for - because you will eventually find one! Question about modified buttock lift after weight loss proceedure-cannot find before/after pics

Congratulations on your weight loss! I would recommend you consider 2 things - ultimately it would depend on the overall goals and your shape (cannot really evaluate the lower back/upper buttocks in your photo). Option one would be a true buttock lift, option 2 would be a thigh lift in the groin crease - I believe given what you've stated that this would address your issue best. To really give you the best answer would require an in person exam or pictures showing the full lower body. I do offer virtual as well as in person consultations.
My insurance won’t pay

An abdominoplasty (skin removal + muscle tightening) is generally not covered by insurance carriers. However, a panniculectomy (skin removal only) can be covered if you meet specific criteria. I perform a lot of body contouring and work closely with my bariatric surgeon counterparts.
Liposuction fat for breast reconstruction?

It could work in the right person. I specialize in these types of reconstruction but not all patients are candidates. A virtual or in-person consultation would be needed to determine if you are an appropriate patient for this procedure.
Silicone in lymph nodes

I have seen this a few times and there is no consensus treatment. The way I have treated these patients is similar to what's been said here: remove the implants and as much extra-capsular silicone as safely possible. I generally let the patient wait a few months to see if the swelling/pain subsides. In only 1 case have I gone back and removed an enlarged, painful lymph node as the rest improved without surgery. You may get different opinions as this is a pretty rare circumstance. Hope this helps!
Uneven/different size breasts

It is entirely too early to tell based on your picture. Give everything at least 3 months to settle and you will get a better (but still not final) idea of where things will be. If you were significantly asymmetric, then a 30 cc difference in implant size will hardly be noticeable. 30 cc is 6 teaspoons (or 2 tablespoons), to put this in perspective!
Lower body lift with augmentation, does this impact the lower front and back thighs?

As a specialist in plastic surgery after weight loss, I would advise against the vertical scar. What I would recommend is a thigh lift using a groin crease incision -- this scar sits on the bikini line in the front and in the upper thigh/buttock fold in the back. For you it will really give shape to the lower buttock/upper thigh area and give definition between the two regions. Check out pictures I posted on my IG @drbodycontouring. I would not recommend a lower body lift in the traditional sense.

Typically I offer 2 options for my patients. Implants (saline or silicone), fat transfer, or a combination. There are advantages and disadvantages to each option, so major sure you know all the answers to your specific questions before proceeding. If you are interested in an abdominoplasty, you would pay for the difference in price (the muscle tightening). PXE disease

PXE is rare and this is not an uncommon issue seen with this disorder. Surgery can help but results are unpredictable in terms of how long they will last and PXE puts you at a higher risk for wound healing complications. I recommend you see a board-certified plastic surgeon with experience in neck/arm lifts. We have a lot of experience with these procedures and would be delighted to see you in the office. What procedure would be right for me

Good evening and thank you for your question, Sounds like you need an abdominoplasty (tummy tuck) but without pictures or a formal exam, it is difficult to say for sure. Also, based on your description, you may have symptoms of panniculitis, and insurance may be able to help cover a portion of this surgery (panniculectomy, which is removal of the skin below the belly button). Good evening, this is something not too uncommon and I have treated several young women with this condition. Typically I recommend a reduction on the heavier side and fat transfer (or implant, but for young patients I recommend no implants) to the smaller side.
