Meet Dr. Lisa Cassileth


I'm Dr. Lisa Cassileth, a board-certified plastic and reconstructive surgeon based in Beverly Hills. I founded The Practice Healthcare more than 20 years ago with a straightforward goal: to bring together surgical excellence and comprehensive wellness care for women under one roof.

While I have expertise across face and body, my focus is the breast and all of its complexity. That includes reconstruction after cancer, revision of prior surgery, explant, and the complicated cases that sit at the intersection of oncology and plastic surgery. In 2015, I founded Bedford Breast Center in Beverly Hills to extend that focus onto comprehensive breast healthcare, including: nipple-sparing mastectomy, minimal-incision lumpectomy, mammography, and genetic testing, all in one place.

 A significant part of my practice involves patients who have been through treatment — surgery, radiation, and chemotherapy — and are now figuring out what comes next. Getting that right requires understanding not just the procedure, but the tissue, the history, and the full picture of what a patient has been through. This naturally transitioned from surgical care into continued care. It's whole-body health for women, under one roof.

I trained at the University of Pennsylvania and served as the first female Chief of Plastic Surgery at Cedars-Sinai Medical Center, where I completed two consecutive terms. I work closely with oncologists, general physicians, radiologists, and referring surgeons. If you have a patient who needs that kind of evaluation, I'm glad to talk you through it.

Your Consultation

A consultation is an opportunity to understand your surgical options in a structured, thorough setting.  You should leave with a clear understanding of your condition, the available approaches, and what each option can realistically achieve.

Consultations typically last 90 minutes to two hours. Valet parking is available in the building garage. After you complete intake paperwork, we will sit down to review your medical history, prior procedures, and current concerns.

If you have had previous breast surgery, I begin by understanding exactly what was done and how your tissue responded. In revision and reconstructive cases, the diagnosis drives the plan. Before recommending another procedure, I need to understand why the prior result did not hold up or did not meet expectations.

I then perform a detailed examination. I assess tissue quality, skin elasticity, implant position when applicable, and any evidence of capsular contracture or other complications. Based on those findings, I will explain the surgical options that are appropriate in your case. We will discuss the operative approach I would recommend, the risks involved, the expected recovery, and what a stable long term result should look like.

Clinical photos are taken for planning and documentation. These images are part of your medical record and guide both operative decisions and postoperative evaluation.

After we meet, you will speak with a Patient Care Coordinator at The Practice Healthcare to review scheduling, postoperative care protocols, and a detailed written cost estimate that includes surgical, facility, and anesthesia fees. You will be able to take that information home and consider it carefully.

Surgery should be undertaken with a clear understanding of the procedure and its implications. If further discussion is useful, we can schedule a second consultation. If questions arise after your visit, my team is available by phone or email to provide clarification.

Breast reconstruction, gel versus saline

Ask your PS to feel saline and the silicone implants, they should have samples at the office. There is also a newer saline implant on the market, called the "ideal"implant, that is far less likely to ripple than traditional saline, but a little firmer than silicone. I let my patients feel the different implants, and most decide that the cohesive gels are safest and most realistic. There is a rare breast cancer called ""BIA-ALCL" that appears to be associated with textured implants, so although unlikely, most patients are choosing the smooth round to avoid the risk. When you swap the expander, ask your surgeon to perform fat grafting. I perform typically 100cc of fat grafting per side, which make s the breasts softer, hides the implant, and warmer! Make sure your surgeon is very comfortable with grafting, as I've found some surgeons perform this more than others.

Fat transfer to the breast is a wonderful procedure, done right! First, the liposuction has to be, of course, amazing. If you have some areas that are hard to lose that fat from , those are perfect, as using that area will allow your breasts to keep their size even with weight loss. I personally love arms and knees, which look great with every bit of fat removed. Next, the grafting must be meticulous,, with the fat suctioned with low suction strength, all oil and tumescent removed, and packaged into small syringes. I use 3cc. THEN, the fat is injected in 0.1 - .3cc passes, with many pinpont access sites. It may take 1,000 passes to correctly place 100cc of breast tissue, but this avoids fat necrosis, which you get when lumps of fat are injected. FINALLY, don't overgraft. A good rule of thumb is you can double your breast size, so make sure that this is what you want. If you are an A and want 400cc implants, it will not be enough. But if you had kids and lost a cup size, perfect!
