Are Silicone implants at 19 possible?!

If you find a plastic surgeon who is willing to place silicone implants “off label” in someone younger than 22, ask them to check if the manufacturer will warranty the. Implant if there is a problem. They may not want to get on the wrong side of the FDA.

Academic setting or private practice?

Physicians at academic medical centers are training residents and fellows who will perform part of your surgery under supervision of the attending physician. All fully trained plastic surgeons were trainees at one time. Most private practice physicians will do your entire surgery themselves with the aide of an assistant. Nowadays most DIEP flaps are being performed in academic medical centers. As you are less than 30 miles from Boston, you should be able to find surgeons there who perform DIEP flaps.

You may want to consider highly cohesive silicone implants which are less likely to ripple whether placed above or below the muscle. Avoiding large implants in women with very little breast tissue also decreases the chance of having visible rippling.

The answer to both of your questions is, probably yes. Apply pressure to the firm area 5 minutes a day using your fingers. It should soften and flatten in 2-3 months. The color may take a year to come back.

I have personally stopped using textured implants 5 years ago to eliminate the very small, but real risk of Anaplastic Large Cell Lymphoma. The most important factors in avoiding displacement of implants is limiting the width of the pocket dissection towards the arm pit and making sure the base diameter of the implant is no more than one centimeter more than your pre-implantation breast base diameter. In your case you should also consider insertion of an absorbable mesh to reinforce the tightening up/repair of the capsule. Good luck.

Unfortunately as the nail bed (the pink tissue under the fingernail) is very thin, a skin cancer of the nail bed may involve the underlying bone. Your first concern should be complete removal of the melanoma in situ.

Capsulectomy necessary with rupture?

If the capsule is not thickened or otherwise abnormal and you do not have textured implants with a late seroma, there is no reason to remove the capsule. Removal of the capsule may distort the breast and increases the risk of bleeding after surgery. Sounds like the advice you received from your plastic surgeon is correct.

I am sorry that you have these problems. You must understand that liposuction is not a weight loss procedure. Most patients who undergo liposuction have no change in their weight. Insurance companies consider liposuction cosmetic and not a covered procedure.

An infection that grew bacteria in contact with an implant is unlikely to be cured without replacing the implant. You need to talk to your plastic surgeon or their coverage if the surgeon is not available.

In the event of staging the surgeries, if the hernia repair is done laparoscopically, I would recommend waiting 2 months to allow the nitrogen gas used during that surgery to be absorbed and the bloating resolved.

Studies show that 60% of women who have undergone breast reduction can successfully breast feed. A similar percentage of women with large breasts who had not had surgery successfully breast fed. I have certainly done reductions on women who were in their early 20’s who had not had children.

In addition to everything my colleagues have said, another possibility is that you may have nasal valve collapse. I see a number of patients who are referred for persistent nasal obstruction in spite of septoplasty (without hematoma), who have this problem. This condition can be diagnosed on examination by your doctor.

It is reasonable to deflate saline implants before deciding on a lift. There is no reason to remove thin capsules. Breast tissue would have to be removed as well increasing the risk for bleeding.
